The commissioner took charge of Tennessee’s Department of Correction in January 2023, after serving as deputy director of Arizona’s corrections department. Hospital staff were working to save her life and, as of late Thursday, were trying to clear the two doses of pentobarbital administered by the state, the lawyers said.
Her legal team said it believed problems with the intravenous lines caused the drug to enter her body rather than her bloodstream. The lawyers welcomed Strada’s resignation but said it would not help Pike and argued that the episode reflected a systemic failure extending beyond one individual. The failed execution has prompted demands for answers from both supporters and opponents of the death penalty.
Tennessee changed its lethal-injection protocol in late 2024 to use pentobarbital alone. Pike was sentenced to death after killing classmate Colleen Slemmer with her boyfriend when Pike was 18.
